fa مقایسه اثربخشی شاک‌ویو و تزریق‌های هدایت‌شده بر درد بیماران مبتلا به سندرم پیریفورمیس: مرور نظام‌مند Comparison of the effectiveness of shockwave and guided injections on pain in patients with piriformis syndrome: a systematic review عمومى General پژوهشی کاربردی Original چکیده مقدمه: سندرم پیریفورمیس یک نوروپاتی فشاری است که در آن عصب سیاتیک توسط عضله پیریفورمیس یا ساختارهای پیرامونی گرفتار می‌شود. هدف این پژوهش، مقایسه اثربخشی درمان با شاک‌ویو در برابر تزریق‌های هدایت‌شده بر بهبود علائم و عملکرد بیماران مبتلا به سندرم پیریفورمیس، از طریق مرور نظام‌مند شواهد بالینی است. روش‌کار‌: این مطالعه به صورت مروری سیستماتیک انجام شد. جستجوی مقالات انگلیسی و فارسی از اوایل تا ٢٠٢5 در پایگاههای خارجی PubMed (MEDLINE)، Web of Science (WOS)، Scopus، Magiran، SID، Noormags و در موتور جستجویGoogle Scholar صورت گرفت و در نهایت 11 مقاله مرتبط بر اساس معیارهای ورود و خروج انتخاب شد. اثربخشی درمان با شاک‌ویو را در برابر تزریق‌های هدایت‌شده، در بهبود علائم و عملکرد بیماران مبتلا به سندرم پیریفورمیس مقایسه‌ و بررسی گردید. پروتکل بررسی در فهرست بین‌المللی بررسی‌های سیستماتیک آینده‌نگر (PROSPERO) با شماره ثبت CRD420251242461 ثبت شد. یافته ها: نتایج این مرور نظام‌مند که بر روی جامعه آماری وسیعی (۵۷۶ بیمار) متمرکز بود، نشان داد که شاک‌ویو برون‌پیکری و تزریق‌های هدایت‌شده با کورتیکواستروئید در درمان سندرم پیریفورمیس ارزش بالینی دارند. هر دو روش در کاهش شدت درد و بهبود کیفیت زندگی مؤثر بوده است. نتیجه‌گیری: شواهد به وضوح تأیید می‌کند که به دلیل اثربخشی مشابه در کاهش درد و ارتقاء کیفیت زندگی بیماران سندرم پیریفورمیس در بازه زمانی تا سه ماه، بین شاک‌ویو برون‌پیکری و تزریق کورتیکواستروئیدها هدایت‌شده، برابری بالینی وجود دارد. مزیت شاک‌ویو برون‌پیکری در غیرتهاجمی بودن و پروفایل ایمنی بهتر است، که آن را به انتخابی اولیه، ایمن و برتر برای افراد دارای ترس از تزریق یا محدودیت‌های درمانی کورتیکواستروئیدها تبدیل کند. Introduction: Piriformis syndrome is a neuromuscular condition characterized by compression or irritation of the sciatic nerve by the piriformis muscle or adjacent structures. Various conservative treatment modalities have been proposed, including extracorporeal shockwave therapy (ESWT) and image-guided corticosteroid injections. This systematic review aimed to compare the effectiveness of ESWT and guided corticosteroid injections in improving pain and functional outcomes in patients with piriformis syndrome. Methods: A systematic review was conducted by searching English- and Persian-language studies published up to 2025 in PubMed/MEDLINE, Web of Science, Scopus, SID, Magiran, Noormags, and Google Scholar. Studies evaluating the effectiveness of ESWT and guided corticosteroid injections in patients with piriformis syndrome were screened according to predefined inclusion and exclusion criteria. Eleven eligible studies involving a total of 576 participants were included in the review. The review protocol was registered in PROSPERO (CRD420251242461). Results: The findings demonstrated that both ESWT and guided corticosteroid injections were effective in reducing pain intensity and improving functional outcomes and quality of life in patients with piriformis syndrome. Across the included studies, the two treatment modalities showed comparable clinical effectiveness, with no substantial differences in short-term outcomes. Improvements were generally maintained for up to three months following treatment. Conclusion: Current evidence supports the clinical equivalence of ESWT and guided corticosteroid injections for the management of piriformis syndrome. Both interventions effectively reduce pain and improve quality of life.
