fa بررسی ارتباط واکسن کووید-19 و شاخص های کمی و کیفی بالینی در بیماران سکته مغزی ایسکمیک Investigating the relationship between COVID-19 vaccine and clinical quantitative and qualitative indicators in ischemic stroke patients تخصصي Special پژوهشی کاربردی Original مقدمه: برای مقابله با همه‌گیری بیماری کووید-۱۹، واکسن‌های متعددی توسعه و تأیید شده‌اند. باتوجه‌به واکسیناسیون گسترده جمعیت جهانی، این مطالعه به بررسی تأثیرات واکسن‌ها بر سیستم عصبی و عوارض ترومبوتیک پرداخته است. هدف اصلی این تحقیق، مقایسه تظاهرات بالینی و نرخ مرگ‌ومیر در بیماران مبتلا به سکته مغزی ایسکمیک واکسینه شده در مقابل بیماران واکسینه نشده بوده است.  روش کار: این مطالعه مقطعی بر روی ۲۵۷ بیمار سکته مغزی مراجعه‌کننده به بیمارستان رجایی کرج در بازه زمانی تیر تا مهر ۱۴۰۱ انجام شد. داده‌ها از طریق چک‌لیست‌های استاندارد و با استفاده از پرونده‌های پزشکی و مصاحبه با بیماران یا همراهان آنها جمع‌آوری گردید. یافته‌ها: تحلیل تک‌متغیره نشان داد که برخی متغیرها مانند سن (۰٫۰۱۵=P). ، کاهش تعداد پلاکت‌های خون (۰٫۰13=P)، هایپرلیپیدمی (۰٫۰۵۶=P)، مصرف داروهای خاص (انسولین ۰٫۰۴۴=P، آتورواستاتین ۰٫۰۱۵=P و کلوپیدوگرل ۰٫۰۳۰=P) و همچنین ازدست‌دادن هوشیاری (۰٫۰۳۷=P) ممکن است با پذیرش واکسن مرتبط باشند. در تحلیل لجستیک چندمتغیره، تنها هایپرلیپیدمی به‌عنوان عامل مؤثر و معنادار آماری شناسایی شد، به‌طوری که بیماران دارای هایپرلیپیدمی نسبت به سایر بیماران احتمال کمتری برای پذیرش واکسن داشتند (021/0=P و 198 /0=OR)، سایر متغیرها ارتباط آماری معناداری با پذیرش واکسن نشان ندادند. نتیجه‌گیری: نتایج این مطالعه نشان داد که در میان بیماران مبتلا به سکته مغزی ایسکمیک، وجود هایپرلیپیدمی به‌طور معناداری با پذیرش واکسن کووید-۱۹ ارتباط دارد. با توجه به اهمیت پیشگیری از عفونت کووید-۱۹ در این گروه پرخطر، شناخت عوامل مؤثر بر تصمیم‌گیری بیماران در خصوص واکسیناسیون می‌تواند در تدوین سیاست‌های آموزشی و مداخلات هدفمند مؤثر واقع شود. Introduction: Following the emergence of Coronavirus Disease 2019 (COVID-19), several vaccines were developed and authorized for emergency use worldwide. Given the widespread administration of these vaccines and concerns regarding potential neurological and thrombotic adverse events, it is important to evaluate their relationship with cerebrovascular diseases. This study aimed to investigate the clinical characteristics and mortality rate of vaccinated patients with ischemic stroke and compare them with unvaccinated patients. Methods: This cross-sectional study was conducted on 257 patients with ischemic stroke admitted to Rajaei Hospital, Karaj, Iran, between July and October 2022. Data were collected using a structured checklist based on information obtained from medical records, patients, and their companions. Demographic and clinical characteristics, vaccination status, and outcomes were recorded and analyzed. The study was approved by the Ethics Committee of Alborz University of Medical Sciences (IR.ABZUMS.REC.1401.269). Results: Univariate analysis demonstrated that age (P = 0.015), thrombocytopenia (P = 0.013), hyperlipidemia (P = 0.056), use of insulin (P = 0.044), atorvastatin (P = 0.015), clopidogrel (P = 0.030), and loss of consciousness (P = 0.037) were associated with COVID-19 vaccine acceptance. In multivariable logistic regression analysis, hyperlipidemia remained the only statistically significant factor associated with vaccine acceptance. Patients with hyperlipidemia were less likely to receive the COVID-19 vaccine than those without hyperlipidemia (P = 0.021, OR = 0.198). No other variables showed a significant independent association with vaccine acceptance. Conclusion: Hyperlipidemia was significantly associated with COVID-19 vaccine acceptance among patients with ischemic stroke.
