fa توفاسیتینیب و پیامدهای قلبی عروقی در بیماران روماتوئید آرتریت: یک مرور سیستماتیک و متاآنالیز به‌روز شده Tofacitinib and Cardiovascular outcome in Rheumatoid Arthritis Patients: An Updated Systematic Review and Meta-Analysis تخصصي Special پژوهشي Research مقدمه: روماتوئید آرتریت یک بیماری التهابی مزمن است که به طور عمده مفاصل را درگیر می‌کند و می‌تواند کیفیت زندگی بیماران را تحت‌تأثیر قرار دهد. توفاسیتینیب به‌عنوان یک داروی مهم در مدیریت این بیماری مطرح است. بااین‌حال، نگرانی‌هایی مبنی بر عوارض جانبی این دارو، به‌ویژه اثرات قلبی - عروقی، همچنان وجود دارد. روش کار: پایگاه‌های داده PubMed ، Embase و Cochrane Central بدون محدودیت زمانی برای مقالات مرتبط مورد بررسی قرار گرفتند. کیفیت مقالات با استفاده از ابزار کاکرین مورد ارزیابی قرار گرفت. داده‌ها با مدل تصادفی و با استفاده از نرم‌افزار CMA نسخه 3 مورد ارزیابی آماری قرار گرفتند. یافته ها: مطالعه حاضر بر اساس داده‌های پنج مطالعه کلینیکال ترایال با مشارکت 3111 بیمار مبتلا روماتوئید آرتریت انجام شد. در گروه مصرف کنندگان دوز 5 میلی گرم توفاسیتینیب، 8 نفر از 1897 بیمار، عوارض قلبی - عروقی را نشان دادند (نسبت شانس برابر با 2.95 با بازه اطمینان 95% از 0.5 تا 17.1). در گروه مصرف‌کنندگان دوز 10 میلی‌گرم توفاسیتینیب، تنها 4 نفر از 1,895 نفر، این عوارض را نشان دادند (نسبت شانس برابر با 1.83 با بازه اطمینان 95٪ در بازه‌های 0.3 تا 9.3). نتیجه‌گیری: توفاسیتینیب به عنوان یک داروی مؤثر در کنترل علائم روماتوئید آرتریت شناخته می‌شود، اما نتایج ما نشان می‌دهد که ممکن است مصرف این دارو با افزایش خطر عوارض قلبی-عروقی همراه باشد. اگرچه این تفاوت‌ها از نظر آماری معنی‌دار نبودند، اما اهمیت نظارت دقیق تر بر مصرف‌کنندگان توفاسیتینیب را نشان می دهد. همچنین تصمیم‌گیری‌های بالینی باید با در نظر گرفتن عوارض قلبی-عروقی انجام شود و اجرای مطالعات با حجم نمونه بیشتر و پیگیری طولانی تر ضروری می باشد.   Introduction: Rheumatoid arthritis is a chronic inflammatory disease that predominantly affects the joints and can significantly impact the quality of life of patients. Tofacitinib is recognized as a key medication in managing this condition. However, concerns about the potential side effects of this drug, especially its impact on cardiovascular health, still exist. Methods: International databases including PubMed, Embase, and Cochrane Central were searched for relevant articles. The quality of the studies was assessed using the Cochrane risk of bias assessment tool. Data were statistically analyzed using a random-effects model and the CMA software version 3. Results: This study, based on data from five trials involving 3,111 rheumatoid arthritis patients, investigated the relationship between Tofacitinib at 5 mg and 10 mg doses and cardiovascular events. In the 5 mg Tofacitinib group, 8 out of 1,897 patients experienced cardiovascular events (odds ratio of 2.95 with a 95% confidence interval from 0.5 to 17.1). In the 10 mg Tofacitinib group, only 4 out of 1,895 patients reported these events (odds ratio of 1.83 with a 95% confidence interval ranging from 0.3 to 9.3). Conclusion: Tofacitinib is recognized as an effective drug in controlling the symptoms of rheumatoid arthritis. However, our results indicate that the use of this drug may be associated with an increased risk of cardiovascular events. Although these differences were not statistically significant, they emphasize the importance of closer monitoring of patients taking Tofacitinib.
