fa بررسی متوسط مدت زمان بین بستری و جراحی و ترخیص بیماران ارتوپدی با شکستگی پری تروکانتریک در بیماران بالای ۶۰ سال در بیمارستان شهید مدنی در سال ۱۴۰۱ Investigating the average duration between hospitalization and surgery and discharge of orthopedic patients with peritrochanteric fracture in patients over 60 years old in Shahid Madani Hospital in 2022 تخصصي Special پژوهشی کاربردی Original زمینه و اهداف: به طور معمول بیماران دارای شکستگی های فمور از جمله پری تروکانتریک دارای کوموربیدیتی های همراه هستند که موجب افزایش مدت بستری، افزایش میزان عوارض و افزایش مورتالیتی می شود. هدف از مطالعه حاضر تعیین متوسط مدت زمان بین بستری و زمان جراحی و ترخیص بیماران بالای ۶۰ سال دارای شکستگی پری تروکانتریک در بیمارستان شهید مدنی در سال ۱۴۰۱ بود. مواد و روش ها: مطالعه حاضر یک مطالعه مقطعی می باشد. اطلاعات دموگرافیک، بیماری های زمینه ای BMI,، مدت زمان بین بستری و جراحی، جراحی و ترخیص و بستری و ترخیص جمع آوری شد .بررسی ارتباط بین فاکتور های پروگنوستیک و مدت زمان بستری انجام شد و سطح معنی داری p value کمتر از 05/0 در نظر گرفته شد.  یافته ها: مجموعا ۲۵۰ بیمار( ۱۱۴ زن و ۱۳۶ مرد) با میانگین سنی 98/0± 6/75سال وارد مطالعه شدند . میانگین مدت زمان بین بستری و جراحی ،8/23± 7/60 ساعت، مدت بین جراحی و ترخیص 4/24± 3/63 ساعت و مدت زمان بین بستری و ترخیص 9/35±1/124 ساعت بود .در جنسیت مونث، سن بالای ۷۵ سال و ابتلا به دیابت، بیماری قلبی عروقی،پرفشاری خون و بیماری تنفسی مدت زمان بین بستری و جراحی به طور معناداری بالاتر بود. در جنسیت مونث، سن بالای ۷۵ سال، افزایش BMI و ابتلا به دیابت و بیماری تنفسی مدت زمان بین جراحی و ترخیص به طور معناداری بالاتر بود. به طور کلی افزایش مدت زمان بین بستری و ترخیص با جنسیت مونث، سن بالای ۷۵ سال، افزایش BMI و ابتلا به دیابت، بیماری قلبی عروقی، پرفشاری خون و بیماری تنفسی به طور معناداری در ارتباط بود . نتیجه گیری: انجام این مطالعه موجب شناسایی ریسک فاکتور های قابل تعدیل در بیماران شد تا از این طریق بتوان با کاهش عوارض شکستگی و جراحی و همچنین کاهش مدت زمان بستری بیماران باعث افزایش شانس بهبودی و برگشت سریعتر آن ها به زندگی روزمره شد. Background and objectives: Usually, patients with femur fractures, including peritrochanteric fractures, have associated comorbidities, which increase the duration of hospitalization, increase the rate of complications, and increase mortality. The aim of this study was to determine the average time between hospitalization and surgery and discharge of patients over 60 years old with peritrochanteric fracture in Shahid Madani Hospital in 2022. Materials and methods: The current study is a cross-sectional study. Demographic information, underlying diseases, BMI, length of time between hospitalization and surgery, surgery and discharge, and hospitalization and discharge were collected. The relationship between prognostic factors and length of hospitalization was investigated and the significance level of p value was less than 0. 05 was considered. Findings: A total of 250 patients (114 women and 136 men) with an average age of 75.6 ± 98 years were included in the study. The average time between hospitalization and surgery was 60.7 ± 23.8 hours, and the time between surgery and discharge was 24. 63.3 ± 4 hours and the time between hospitalization and discharge was 124.1 ± 35.9 hours. In female gender, age over 75 years and suffering from diabetes, cardiovascular disease, hypertension and respiratory disease, the time between hospitalization and Surgery was significantly higher. In female gender, age over 75 years, increased BMI, diabetes and respiratory disease, the duration between surgery and discharge was significantly higher. In general, increasing the length of time between hospitalization and discharge was significantly related to female gender, age over 75 years, increased BMI and diabetes, cardiovascular disease, hypertension, and respiratory disease.
