fa مقایسه تاثیر بکارگیری والوون و رینگر درمحلول پرایم پمپ قلبی ریوی بر میزان خونریزی پس از عمل و عملکرد کلیوی در اعمال جراحی پیوند عروق کرونری قلب Comparison of the Effect of Voluven and Ringer Solution in Cardiopulmonary Bypass on the Bleeding After the Surgery and Renal Function in Coronary Artery Bypass Graft Surgeries تخصصي Special پژوهشي Research <div> <div><div id="_com_1" uage="JavaScript"></div></div> </div> مقدمه: دراعمال جراحی قلب باز نیاز به دستگاه بای پس قلبی ریوی cardiopulmonary bypass(CPB) وجود دارد. برای آماده سازی دستگاه قلب وریه که در بسیاری از جراحی‌هایی که روی قلب انجام می‌گیرد بکار میرود، نیاز به محلول پرایم است. برای پر کردن مخزن وریدی و لوله با مایع و زدودن ذرات هوا وسایر ذرات احتمالی موجود در خطوط و سیستم، حدود 1500-2000میلی لیتردر بیماران بزرگسال مایع پرایم مورد نیاز است. هدف این مطالعه مقایسه تاثیر بکارگیری والوون و رینگر درمحلول پرایم پمپ قلبی ریوی بر میزان خونریزی پس از عمل و عملکرد کلیوی در اعمال جراحی پیوند عروق کرونری قلب می باشد. مواد و روش‌ها: این مطالعه از نوع مداخله بالینی تصادفی شده می باشد. در این مطالعه دو نوع محلول پرایم یعنی رینگر به عنوان کریستالوئید و والوون (هیدروکسی اتیل استارچ6%)به عنوان کلوئید در دو گروه جداگانه مورداستفاده قرار می‌گیرد و با هم از نظر میزان بروز خونریزی و عملکرد کلیوی در 24 ساعت اول بعد از عمل در آی سی یو جراحی قلب مقایسه شدند. نتایج: میزان درناژ 24ساعته (خونریزی24ساعته)در آی سی یو ازنظر آماری دارای تفاوت معنی‌دار نبودند (05/0 < P). میزان برون ده ادراری در دو گروه در دو <a>بازه</a>  زمانی یعنی در مدت زمان پمپ ودر طی 24ساعت اول بعد از ورود بیمار به آی سی یو تفاوت معنی‌داری نداشتند. میانگین partial tromboblastin time postop (PTT بعد از عمل)در گروه <a>رینگر به طور معنی داری بیشتر از گروه </a>والوون <a>بود </a> (05/0 > P). سطوح پلاسمایی Bun-Cr، سدیم، پتاسیم، هموگلوبین، هماتوکریت، پلاکتBT،PT ، INR، 24 ساعت بعد از عمل تفاوت معنی‌داری نداشتند (05/0 < P). <a>میزان نیاز به تزریق خون در حین عمل و طی</a> 24ساعت بعد ازعمل بین دو گروه تفاوت معنی‌داری را نشان می‌دهد  (رینگر 9/0 ± 70/1 و والوون 1/1 ± 07/1) (019/0 = P ). پروتئین ادرار در این دو گروه در 24 ساعت بعد از عمل تفاوت معنی‌داری نداشت (08/0 = P). نتیجه گیری: براساس یافته‌های تحقیق می‌توان نتیجه گرفت که استفاده از والوون در محلول پرایم باعث اختلاف چندانی در خونریزی بعد از عمل نمی شود <a>با این حال </a> نیاز به تزریق خون در این بیماران کمتر می‌شود. <div> <hr align="left" size="1" width="33%" > <div><div id="_com_1" uage="JavaScript"></div></div> </div> Background: This study compared the effect of Voluven and Ringer solution in cardiopulmonary bypass on the bleeding after the surgery and renal function in coronary artery bypass graft surgeries. Methods: Two types of prime solution including ringer - as Crystalloid- and Voluven were used in two groups and compared in terms of bleeding and renal function during the first 24 hours after the surgery in intensive care unit (ICU) cardiac surgery. <a>Results:</a> Amount of a 24-hour drainage was not statistically different (P > 0.05). There was not any significant difference between groups regarding urinary output in two time periods, pump time and during the first 24 hours after the patient’s entry to ICU. The mean of partial thromboplastin time postop in experiment and control groups was significantly different (P > 0.05). Plasma level of <a>Bun-Cr</a>  , sodium, Potassium, hemoglobin, hematocrit, <a>BT, PT </a>and INR Platelets was not significantly different during the first 24 hours after the surgery (P > 0.05). There was a significant difference between the two groups in terms of a <a>need for blood transfusion </a>  during the surgery and first 24 hours after the surgery (Ringer 1.70 ± 0.9, Vovuven 1.07 ± 1.1) (P = 0.019). There was not a significant difference between two groups in terms of urinary protein secretion during the first 24 hours after the surgery (P = 0.08).
