Introduction: Root canal treatment is a fundamental component of dental practice that aims to preserve natural teeth and maintain their long-term function. However, procedural errors and iatrogenic events may occur during treatment, potentially affecting treatment outcomes and prognosis. Identifying the frequency and causes of these events is essential for improving clinical training and reducing treatment-related complications. This study aimed to evaluate the frequency of iatrogenic endodontic events during root canal treatment performed by dental students at Alborz Dental School between 2020 and 2022.
Methods: This descriptive-analytical cross-sectional retrospective study was conducted using the records of patients treated in the Department of Endodontics at Alborz Dental School from 2020 to 2022. A total of 428 patient records that met the inclusion criteria were reviewed. Data regarding procedural errors and relevant variables were collected using a standardized checklist completed by students and faculty members. Statistical analyses were performed using the chi-square test in SPSS version 24. Ethical approval was obtained from the Ethics Committee of Alborz University of Medical Sciences (IR.ABZUMS.REC.1400.153).
Results: Among the 428 reviewed cases, approximately 30% were associated with at least one procedural error. The most common errors were voids in root canal obturation (36.6%) and overfilling (25.9%). No significant associations were found between the occurrence of errors and students’ sex, age, or educational level (P > 0.05). However, significant associations were observed between procedural errors and tooth type, dental arch, and practical endodontic course level (P < 0.05). The highest frequency of errors occurred in mandibular molars and during the first and third practical endodontic courses.
Conclusion: Most root canal treatments performed by dental students were completed without procedural errors. Voids and overfilling were the most frequent iatrogenic endodontic events. Mandibular molars and certain stages of clinical endodontic training were associated with a higher incidence of errors. These findings highlight the need for targeted educational interventions to improve students’ endodontic skills and treatment outcomes.
