fa جا به جایی چرخشی مفصل آتلنتوآگزیال در همراهی با شکستگی تیپ 2 ادنتویید در ترومای با انرژی بالا : گزارش موردی و بررسی مقالات Rotational dislocation of the atlantoaxial joint associated with type 2 odontoid fracture in high energy trauma A case report and review of articles تخصصي Special پژوهشی کاربردی Original مقدمه : همراهی در‌رفتگی مفصل آتلانتواکسیال و شکستگی زائده ادنتوئید ،یک آسیب نادر و جدی ستون فقرات است که باعث ایجاد بی ثباتی می‌شود .فراوانی تخمین زده شده برای این آسیب ترکیبی در میان آسیب‌های ستون فقرات گردنی فوقانی کمتر از 2 درصد است .موارد بالینی کمی از این نوع آسیب در منابع علمی گزارش شده است . در این مقاله، پیش آگهی یک بیمار با در رفتگی مفصل آتلانتو اکسیال و شکستگی زائده ادنتوئید و نحوه مدیریت این نوع آسیب ترکیبی را، ارائه می‌کنیم. گزارش مورد : یک مرد ۱۹ ساله به دنبال تصادف با وسیله نقلیه موتوری دچار شکستگی محور 2 C در زائده ادنتوئید و جا به جایی مهره اطلس (1 (C نسبت به 2C به سمت خلف شده بود.در این بیمار، جابه‌جایی خلفی با در‌رفتگی دو طرفه مفاصل جانبی مشاهده میشد.درمان محافظه‌کارانه شامل کشش محوری با بریس سر halo برای کاهش در‌رفتگی آتلانتواکسیال انجام شد ولی موفقیت آمیز نبود. تصمیم به انجام عمل جراحی برای کاهش در‌رفتگی و ثابت‌ کردن مفصل2C1-C  گرفته شد.عکسبرداری بعد از عمل جراحی رضایت بخش بود.در طول 6 ماه پیگیری، تصاویر بالینی بیمار ثابت و بیمار فاقد مشکل عملکردی ماژور بود .اشعه ایکس داینامیک، دامنه حرکت خوبی را در خم شدن و اکستنشن نشان داد. بحث/ نتیجه‌گیری : همراهی در‌رفتگی مفصل آتلانتواکسیال و شکستگی زائده ادنتوئید، یک آسیب نادر ستون فقرات است که به دلیل ایجاد بی ثباتی ، جدی تلقی می‌شود. ابتدا اقدام به درمان در‌رفتگی می‌کنیم و سپس درمان شکستگی زائده ادنتوئید بر اساس نتایج جا‌اندازی و پایداری آن تعیین می‌شود. درمان محافظه کارانه در‌رفتگی شامل کشش محوری با بریس سر halo است .درصورت شکست درمان محافظه کارانه باید یک جا‌اندازی باز از طریق روش خلفی با فیکساسیون C1-C2 نوع Magerl انجام شود. اگر این روش امکان پذیر نباشد، فیکساسیون اکسیپیتوسرویکال انجام می شود. این نوع فیکسیشن به ندرت استفاده می شود اما به دلیل قرار گیری عالی مفصل آتلانتواکسیال درآن و امکان انجام فیوژن ترانس آرتیکولار قدامی در این روش، دارای مزیت است. Background: The combination of atlantoaxial joint dislocation and fracture of the odontoid process is a rare but serious spine injury that causes instability. The estimated frequency of this combined  injury is less than 2% among upper cervical spine dameges. Few clinical cases of mentioned damage has been reported in scientific resources. In this article, we present the prognosis of a patient with atlantoaxial joint dislocation along with odontoid fracture and discuss how to manage this type of combined injury. Case presentation: A 19-year-old man presents with a fracture of the C2 axis in the odontoid process and posterior displacement of the atlas vertebra (C1) relative to C2 due to motor vehicle accident. posterior displacement was observed with bilateral dislocation of the lateral joints. Conservative treatment consisted axial traction with a halo head brace was performed to reduce the atlantoaxial dislocation. Because traction failed, we decided to perform surgery to reduce the dislocation and fix the C1-C2 joint.postoperative X-ray and CT scan were satisfactory. At the 6 months follow-up, the clinical picture was stable and the patient had no major functional problems. Dynamic X-ray Showed good range of motion in flexion and extension. Conclusion: The combination of  atlantoaxial joint dislocation and fracture of the odontoid process is a rare spine injury that is considered serious due to causing  instability. First, we treat the dislocation, then the treatment of the fracture of the odontoid appendage is determined based on the results of displacement and its stability. Conservative treatment of dislocation includes axial traction with a halo head brace. If traction fails, an open reduction should be performed through the posterior approach  with C1-C2 Magerl fixation. If this approach is not possible, occipitocervical fixation is performed.
