تشخیص عفونت هلیکوباکترپیلوری در کودکان و بزرگسالان با استفاده از تست سرولوژیک و تست آنتیژن مدفوعی

زمینه وهدف: عفونت با هلیکوباکتر پیلوری عامل ایجاد گاستریت مزمن مرتبط با زخم دئودنوم، زخم معده و احتمالا آدنوکارسینومای معده است. تستهای تشخیصی غیرتهاجمی شامل تست اوره تنفسی و تستهای سرولوژیک و آنتیژن مدفوعی میباشند. تشخیص سرولوژیک عفونت هلیکوباکتر پیلوری در کودکان صحت بالایی ندارد. به منظور بررسی پاسخ ایمنی به هلیکوباکتر پیلوری در کودکان و بزرگسالان، آنتیبادیهای IgG و IgA ضد هلیکوباکتر پیلوری را با آنتیژن هلیکوباکتر پیلوری در مدفوع (HpSA) مقایسه کردیم. مواد و روشها: نمونه مدفوع و سرم از 218 بیمار کودک و بزرگسال مراجعه کننده به بیمارستان امام حسن مجتبی که دارای علائم کلینیکی بوده و در محدوده سنی 4 تا 77 سال قرار داشتند جمعآوری شد. نتایج سرولوژیک (IgG و IgA) و HpSA با استفاده از روشهای ایمنواسی بدست آمدند. یافتهها: از 218 نتیجه سرولوژیک و HpSA بدست آمده، 39 مورد مربوط به کودکان (≤17 سال) و 179 مورد مربوط به بزرگسالان (≥18 سال) بود. میزان نتایج مثبت مربوط به HpSA (8/45%) بطور مشخص (P<0.001) کمتر از نتایج مربوط به IgG (6/54%) و بیشتر از IgA (9/28%) بود. همچنین ویژگی تستهای سرولوژیک IgG و IgA در کودکان درمقایسه با بزرگسالان بیشتر بود. نتیجهگیری: در این مطالعه HpSA بعنوان یک ابزار کلینیکی و اپیدمیولوژیکی حساس و اختصاصی جهت ارزیابی عفونت هلیکوباکتر پیلوری استفاده شد. IgG همبستگی بهتری با HpSA نسبت به IgA داشت و همچنین IgG درکودکان بسیار حساستر از بزرگسالان بود که موید این مطلب، احتمال تماس بیشتر بزرگسالان با هلیکوباکتر پیلوری در گذشته است. با استفاده از HpSA بعنوان استاندارد طلایی ما مشاهده کردیم که عملکرد تستهای سرولوژیک IgG و IgA نسبت به سن متفاوت است، زیرا پاسخ ایمنی ناکامل یا تحمل به هلیکوباکتر پیلوری در کودکی وجود دارد و بنابراین تشخیص سرولوژیک هلیکوباکتر پیلوری کاربرد کمتری خواهد داشت. بنابراین ما پیشنهاد میکنیم که آزمایشگاهها محدوده تیترهای سرولوژیک خود را بر اساس سن بازنگری کنند درحالیکه همبستگی کلینیکی بیشتر در این زمینه جهت تثبیت محدودههای اپتیمم لازم است.
Background: Helicobacter pylori infection causes chronic gastritis that is related to duodenal ulcer, gastric ulcer, and possibly gastric adenocarcinoma. Noninvasive diagnostic tests consist of the urea breath test, serology, and stool antigen testing. Serodiagnosis of H.pylori infection is inaccurate for children. In order to investigate the immune response to H.pylori in children and adult, we compared anti-H pylori IgG and IgA antibodies with H. pylori antigen (HpSA) in the stool. Methods: Serum and stool samples were obtained from 218 children and adult patients with clinical symptom in the range of 4 to 77 years old. Paired results of H. pylori serology (IgG and IgA) and HpSA were analyzed by enzyme immunoassay methods. Results: There were 218 paired serology and HpSA results for 39 children (≤17 years) and 179 adult (≥18 years). The positivity rate of HpSA (45.8%) was significantly lower (P<0.001) than those for H. pylori IgG (54.6%) and IgA (28.9%). Moreover in child patients specificity for serological test IgG and IgA were higher than adult. Conclusion: In this study, HpSA was sensitive and specific as a clinical and epidemiological tool to evaluate H. pylori infection. IgG correlated better with HpSA than IgA, and also IgG was much more specific in children than adults confirm the fact that adults are more possible to have been exposed to H. pylori in the past. Using HpSA as the gold standard, we found that the performances of IgG and IgA serology tests differ significantly by age because immature immune response or tolerance to H. pylori is present in childhood and serodiagnosis of H. pylori infection is less useful. Hence, we recommend that laboratories reevaluate reference serologic titers based on age and further clinical correlation is needed to establish the optimal ranges.
